As stated above there are several different areas you can lose from, which is why many fad diets don't work. They don't target fat loss, so they seem effective, but you quickly gain it back. this leads many people to believe dieting is hopeless.
Starvation / Extreme low cal diets (anorexia), here you lose a lot of muscle mass, so even though you drop weight. So technically you can get fatter while losing weight.

Proper nutrition and exercise is the only sure fire way to lose fat short of surgery.

If you mean treatment centers like that, they usually either have eight hour shifts like usual, or more commonly , long overnight hours, like friday, sat , sunday, or three to four day work weeks, then time off.
the pay is usually low and unrewarding……..minimum wage to low salary wages
the work and hours are tiring and unrewarding in general
dont recommend it for long term work, only to build experience.

You need to contact the nearest hospital's social services department, ask for a social worker and pose this question to them.
Alternately, call the pediatrician, or the neurologist who made the diagnosis, or provides ongoing care and ask to speak to the social worker that they deal with.

If you get too frustrated, walk away. Short term abandonment is better than physical discipline for someone who doesn't understand what they have done wrong.
Don't hit a child, don't shake a child, don't scream at a child. Simply walk out the door, take some deep breaths until you gain your composure. Call a friend or relative, knock on the next door neighbor's door, tell them you are about to snap, and need them to watch your child for 20 minutes or so, until you can calm down.

Good lord no, anaesthetic isn't something you want to go messing around with. Just bite the bullet and deal with the pain, depending on the size of the design and its placement it might not hurt too much anyway.
My most painful was on the back of my knee. I admit that I had to bite on my hand throughout the experience, fortunately it was when I was young and dumb so it was a tiny piece and only took 20 minutes.
